In a series of blogs beginning in January, you were asked to rate how motivated you were to make a change. You were asked how confident you were to make a change. You were challenged to dig deeper and decide what area of change you would be focusing on for the next three months. 

Now you are asked: Where are you now and where do you want to be?

The first step is to ask "How do you picture your best health? What would you like to feel and look?"

The next step is to rate each of the following on a scale of 1 (low) to 10 (high).

  • Nutrition  ______

  • Personal Development ______

  • Professional Development _______

  • Your Physical Home and/or Workplace Environment _______

  • Your Family, Friends, and/or Co-worker Relationships ________

  • Your Communication Skills ______

  • Mind-Body Connection (how you relax and heal) ______

  • Prevention and Interventional Care (routine heal screenings such as dental exams, vitamins, supplements, colonoscopies) ______

  • Spirituality ______

If nothing changes in your health, what would be the worst-case scenario?

If you make significant health changes, what would be the best-health scenario?
